IRS Contribution Letter for Monetary Donations: Click here for the letter you need to fill in to claim a monetary contribution to the team. All families paid a tax-deductible contribution of $175 to the team as part of the initial registration fees.
IRS Contribution Letter for In-Kind Donations: Click here to find the letter you can use to document in-kind contributions to the team.
Check Request Form: Click here for the check request form. If you have a pre-approved expense for which you wish to be reimbursed, fill in this form and give it, with your receipts, to the team treasurer, Lisa O'Hearn.
Registration Forms: Remember, if you swam for the team last season, you do not need to fill out the forms again - just review/update your data in the Registration Binder during registration week. You should, however, fill out a new Emergency Treatment Form at the start of the 2010/11 season.
